Open Breeding Poultry Entry Form

Please fill out the following Entry Form in its entirety. Please note: you will need to attach/upload a completed Entry Details Form including information on breeds, variety and sex of the birds. The Entry Details Form is available on our Livestock/Open Shows page below the Breeding Poultry Show Handbook and Rules. Please TITLE/Name the Detials form with your Last Name and Breeding Poultry Page number in the title as listed on the form.

REGISTRATION AND FEES INFORMATION:
Registration/entry fees for the Open, Junior or Backyard Show will be $7.00 per bird. Double coops will be $14.00.
Entry deadline will be entered by October 1, 2021 through online. No onsite day of show entry will be allowed.
Payment due at time of Registration online.
